How to Document Your Wishes

During these uncertain times, many are rushing to document their wishes. Here are three free documents from Carrier Law to help you do so.

Care Directive Questionnaire

Also known as an “Advance Directive” or “Living Will”, this document outlines your wishes for decisions around your care, burial, and funeral.

Funeral Representative Designation

This document designates a representative to make decisions about your funeral arrangements after your death.

HIPAA Form

This document designates who you would like to have access to your medical records, billing records, and any other information protected by the Health Insurance Portability and Accountability Act.

Please note that the above documents do not act in place of power of attorney documents for medical or financial decisions, nor do they give rise to an attorney-client relationship. Power of attorney documents are perhaps the most critical documents to protect you and your family, and avoid probate court in a time of crisis. They should be drafted by an experienced Estate Planning attorney.
